Achalasia cardia is a motility disorder where there aperistalsis of the body esophagus and failure lower esophageal sphincter relaxation. This manifests as functional gastroesophageal junction obstruction. rare has prevalence 10 in 100,000 population. It peak incidence between 30-60 years age. The pediatric population very low even rarer infants. Here we present case achalasia 9-month-old male infant weighing 3.1 kilograms. child had complaints regurgitation milk, non-projectile vomiting, fever cough for past 3 months along with to thrive. He was treated bronchopneumonia initially then evaluated barium swallow. swallow revealed hold-up contrast distal typical bird-beak appearance. Definitive surgical intervention being planned. However, aspirated expired. Did delay procedure? Were too late? These questions continue make us self-introspect on management infantile cardia. paucity available publications data major roadblock management.
